Intake form and waiver

Let's get the paperwork out of the way.

IMPORTANT NOTICE — READ BEFORE BOOKING 

While your safety — and the safety of others — is our priority, participation in helicopter skiing, remote mountain travel, and related activities involves inherent, significant, and unavoidable risks, including the risk of serious injury or death. 

All persons participating in activities with PNH must: 

  • Follow all PNH policies and operational procedures. 

  • Comply with the directions and decisions of PNH guides and staff. 

  • Attend required safety briefings and trainings. 

 

Visiting PNH and participating in our activities is entirely voluntary. Because participation is a choice, signing the Waiver is also up to you and the other participants on your reservation. If you or your guests do not feel comfortable signing the Waiver, you may simply choose to not participate in voluntary recreational activities with PNH. However, if you do choose to join us, every participant is required to review, sign, and agree to be legally bound by PNH’s Participant Release of Liability, Waiver of Claims, Assumption of Risk, and Indemnity Agreement (“Waiver”). The Waiver is available for your review prior to booking. The Waiver includes provisions relating to your release of liability; assumption of inherent risks; indemnification obligations; limitations on claims; dispute resolution procedures; and other legal matters of significance.  

If you are booking or providing payment on behalf of another participant, you acknowledge and agree that you are responsible for ensuring that each participant in your party receives access to the Waiver and understands that signing the Waiver is required prior to participation. 

If you are booking or paying for a minor, you confirm that you are the minor’s parent or legal guardian and agree to review and sign the Waiver on the minor’s behalf. 

 

Please note that your reservation remains unconfirmed, and your payment fully refundable, until we receive your signed Waiver.
